Abstract

背景技术
目前,在美国,因结肠癌导致的死亡每年占恶性肿瘤死亡人数的11%。每10万人中即有62人患此疾病,最流行时每10万人将有300人患此疾病。该疾病是美国男性死亡的第三大病因,是女性死亡的第四大病因。因晚期诊断的结肠癌患者只有50%的非常可怜的5年存活率。外科手术和化学治疗是目前较好的治疗方法,它们不能提高存活率。需要对已知有发展为结肠癌的较大的危险的患者群体进行早期安全、有效的预防性治疗的方法。
类花生酸和胃肠细胞的分化功能

作为结肠癌的前兆的息肉和畸变的小囊病灶
目前普遍认为,腺瘤息肉是直肠结肠癌(Colorectal Cancer)的前兆,其外观、大小和重复出现将预示着发展为结肠癌的相对危险(参见Lotfi et al.(1986)

(4)抗氧化性质-结肠特异性抗氧化活性进一步有助于降低致癌物负荷。
(5)类似于NSAID的抗炎症机制-活性代谢物应该抑制炎症诱导的类花生酸途径,然而,不损害基本修复途径的类花生酸抑制作用会是优选的。
总之,某些NSAIDS已表现出抑制致癌物的诱导的模式动物结肠肿瘤发生率及抑制人体息肉生长。虽然NSAIDS对肿瘤抑制的机制尚未得到证明,但这些药物可以调节胃肠类花生酸的产生和代谢。不幸的是,NSAIDS对胃肠有严重的副作用,从而妨碍了这些药物的正常使用。因此,寻找一种作为有效的化学预防剂而又没有副作用的NSAIDS将是非常合乎需要的。Chan的美国专利4,412,992描述了2-羟基-5-苯基偶氮苯甲酸衍生物的制剂及其在治疗溃疡性结肠炎中的用途。

图3表示5-ASA在0,0.1,1.0和10mM的剂量下对腺癌细胞系LS174T增生的作用效果。图中所示的细胞数为三次重复实验的平均值。
图4为在有或无balsalazide存在下用饮用水中的致癌物氧化偶氮基甲烷处理大鼠群体诱导的畸变结肠小囊数量的分析结果。
图5表示在低浓度下balsalazide和5-ASA的相对抑制反应。
图6A和6B分别表示对照动物和用AOM(20mg/kg)注射动物(第二次注射后6周)的整个结肠中畸变小囊病灶的分布情况。示出了含有1,2,3,4,或5或更多的病灶数量分别在每格从下至上的线中。
图7表示在暴露于细胞之前4天以各种浓度溶于介质中的5-ASA(黑色)和在即将暴露于细胞之前以10mM浓度的溶于介质中的5-ASA(白色)产生的对LS174T细胞增生的抑制作用。细胞再生长另外的四天后,进行定量分析。

实施发明的方式
其中X为-SO2-或-CO-基因,R为苯基或羧甲苯基或者为-(CH2)n-Y基团,其中Y为羟基、氨基、单烷基-或二烷基-氨基基团(其烷基部分含有的碳原子数可达6个)或羧基或磺酸基,n是1至6的整数,亚烷基中的一个或多个氢原子可由氨基、单烷基-或二烷基一氨基(其烷基部分含有的碳原子数可达6个)或者烷基取代。其中-(CH2)n-Y基团直接与氮原子相连或者通过苯环与氮原子相连,但R-NH-X-不能为-CO-NH-CH2-COOH基团
此外使用的术语“2-羟基-5-苯基-偶氮苯甲酸衍生物”也包括该化合物的酯类或活性代谢物,或者该化合物或其酯或活性代谢物的非毒性的、药理学上可接受的盐。
术语“活性代谢物”指2-羟基-5-苯基偶氮苯甲酸衍生物在人体中的代谢产物,如通过结肠细菌的作用,该代谢产物可以抑制结肠癌细胞的增生。例如,下面将讨论的5-ASA即为balsalazide的一种活性代谢物。
术语“氧化产物”指把2-羟基-5-苯基偶氮苯甲酸衍生物的活性代谢物(如5-ASA)暴露于象次氯酸盐或过氧化氢的氧化条件下产生的产物。
balsalazide钠盐是一种具有结肠特异性、非类固醇类、抗炎症的氨基水杨酸,在活动性溃疡性结肠炎的治疗方面有用。balsalazide和其一种初级代谢物可抑制培养的人结肠癌细胞的生长,balsalazide抑制用致癌物氧化偶氮基甲烷处理的动物中畸变小囊的形成。象其他的NSAIDs一样,balsalazide在人体直肠结肠癌的化学预防方面的效果。然而,balsalazide有三个重要的安全优点:(1)药物的释放具有结肠特异性;(2)尚没有观察到可引起胃或十二指肠溃疡的迹象和(3)在迄今治疗的500多例病人中,对某些病人进行长达3年的观察,尚未报道有肾脏毒性。因此,balsalazide在结肠癌的化学预防方面具有既有效又安全的理想的结合。
药物的结肠特异性释放:balsalazide为前药,该药物的无活性形式,象Sulindac一样,可通过结肠细菌的作用转变为一种有活性的抗炎症药物。如图1所示,balsalazide通过一个偶氮键把氨基水杨酸盐(5-氨基水杨酸(5-ASA))与一个惰性载体4-氨基苯-β-丙氨酸(4-ABA)相连。细菌的偶氮还原酶可以水解此键,释放5-ASA,产生局部作用。
有限的系统吸收:当口服balsalazide时,通过上部胃肠道不能分解并且本质上也难以吸收。在人体血浆和尿中只发现仅吸收前药剂量的0.3%,99%的药物可完整地到达结肠部位。在结肠中,药物经过水解形成5-ASA和4-ABA,再与结肠粘液相互作用,转变成其N-乙酰形式。balsalazide单独给药产生的绝大部分5-ASA在96小时以后可转变为N-乙酰-5-ASA(NacSASA),并可随尿液排出,而4-ABA及其N-乙酰代谢物则很少被吸收。由于5-ASA的乙酰代谢物为无活性形式,故也可减少释放的5-ASA的系统活性(Chan et al.(1983)

5-ASA的氧化产物在体外抑制结肠癌细胞的增生:与5-ASA相比,5-ASA的氧化产物对人体结肠癌细胞显示出意想不到的生长抑制活性。
结肠癌化学预防和(或)化学治疗的方法:
本发明的2-羟基-5-苯基偶氮苯甲酸衍生物,特别是bal-salazide和其代谢物对结肠癌的化学治疗有用,它们最好以包含2-羟基-5-苯基偶氮苯甲酸衍生物的药物组合物的形式给结肠癌或有发展为结肠癌危险的患者施用。所述药物组合物也可以包含一种或多种非毒性的、药理学上可接受的载体、赋形剂、和(或)稀释剂。口服是优选的。使用标准的药物配方技术,例如,Remington′s Phar-macentical Sciences,Mack Publishing Co.,Easton,PA,Latest edition上描述的方法。
药物组合物可以片剂、胶囊、粉剂、颗粒剂、锭剂、液体或胶状物形式。供口服的片剂和胶囊可以以适于单位剂量用药的形式,并且可以含常规的赋形剂,这些例子有:结合剂如糖浆、阿拉伯树胶、凝胶、山梨醇、黄著胶、聚乙烯吡咯烷酮(PVP);填料如乳糖、糖类、玉米粉、磷酸钙、山梨醇或甘氨酸;片剂润滑剂如硬脂酸镁、二氧化硅、滑石、聚乙二醇或二氧化硅;崩解剂如马铃薯淀粉;可接受的润滑剂如月桂基硫酸钠。片剂可按照已知的常规制药实践中的方法进行包衣。口服液体制剂可以是水状或油状悬浮液、溶液、乳剂、糖浆或酏剂,也可制成一种干物质,在使用之前再用水或其他合适的载体重新调制。这些液体制剂可含有常规的添加剂,例如悬浮剂(如山梨醇、糖浆、甲基纤维素、葡萄糖浆、明胶、经氢化的食用油脂)。乳化剂(如孵磷脂,山梨醇单油酸盐或阿拉伯树胶),非水相载体(包括食用油)(如杏仁油、精馏的椰子油、油脂如甘油、丙二醇或乙醇),防腐剂(如甲基或丙基对羟基苯甲酸或山梨酸),如果需要也可含有常规的风味剂或着色剂。
本发明药物组合物中活性物质的百分比是可变的,因为必须使药物调制成一定合适比例的剂量,以获得理想的疗效。总之,本发明的药物制剂经口头或直肠给药时应按每70公斤体重每天1至14克活性物质施用。以下说明性地而不是限制性地给出一些实施例。
实施例
实施例1:Balsalazide和5-ASA在体外抑制结肠癌细胞增生
在体外用人腺瘤细胞HT-29和LS174T实验证实了Bal-salazide和其代谢物5-ASA对人体结肠癌细胞生长的抑制活性。细胞生长于补有10%的小牛血清的DME(Dulbeccos Modified Ea-gle)培养基中,于含5%CO2的氛围和37℃下培养。为进行生长实验,将细胞接种于24-孔组织培养皿中(2cm2/每孔,每孔中的细胞密度为20,000个),并使其附着于孔中生长24小时,然后再改变培养基,用含试验药物的新鲜培养基代替。
将试验药物以所需的浓度溶于组织培养基中,并加入每个孔中。为了测定每个孔中的细胞数量,除去培养基,并用磷酸缓冲盐溶液冲洗细胞,然后再用胰蛋白酶-EDTA溶液培养细胞15分钟,直至细胞脱落。再从孔中移出脱落的细胞,用Coulter计数器(ZBI模型)进行细胞计数。如图2和图3所示,两种人体结肠癌细胞系HT-29和LS174T的生长随5-ASA浓度的升高而受到抑制。当5-ASA的使用剂量为10mM时,生长抑制程度分别为82%和85%。
比较研究子balsalazide及其两种代谢物5-ASA和4-ABA,以及乙酰水杨酸(阿司匹林)对癌细胞增生的作用效果。如表1所示,乙酰水杨酸共价(Covalently)抑制环加氧酶,在该试验中对癌细胞增生有强烈的抑制作用。母体药物balsalazide的抑制作用也是如此。然而,balsalazide的载体分子对癌细胞增生却没有作用。表I中所示的细胞数量为三次重复培养孔中的细胞平均数。

如图5所示,较低浓度的balsalazide比5-ASA始终产生更为明显的抑制反应。用HT-29细胞实验时,IC50值分别为5-7mM和11.8mM,而用LS174T细胞实验时,IC50值则分别为5.2mM和11.5mM。在5-ASA的最低试验浓度条件下(对LS174T为0.1-1.0mM,对HT-29则为0.1-4mM),对两种细胞观察到可重复的统计学上显著增加的细胞数量(增加约20%)。这与以前其他人发表的研究结果一致,他们观察发现,较低浓度的其它NSAIDs可以诱导蛋白质合成的增加,而较高浓度的NSAIDs却抑制蛋白质的合成(Hial et al.(1977)

实施例2:balsalazide和代谢物对大鼠畸变小囊形成体的影响。
在致癌物诱导的结肠粘膜中,畸变小囊最初被描述为“体积增大、上皮变厚、小囊周区增大”(Bird et al.(1989)

8:189-200)。NSAIDs是大鼠体内致癌物氧化偶氮基甲烷(AOM)诱导畸变小囊形成的最强的抑制剂之一(Wargovich et al.(1992)

16G:51-54)在用AOM长期对动物研究中已确证这些因子的抗肿瘤活性。因此,畸变小囊模型对体外研究balsalazide和代谢物的效能是理想的。
对畸变小囊诱导的抑制作用
在大鼠上氧化偶氮基甲烷(AOM)诱导的结肠癌试验模型中,测定了balsalazide(BSZ)和其它代谢物5-氨基水杨酸(5-ASA)和4-氨基苯-β-丙氨酸(4-ABA)对畸变小囊体形成的抑制效能。
使用四组雄性F344大鼠,每组5只。大鼠为6-8周龄,开始的体重在总平均体重的+/-20%范围之内。按已发表的研究方法用AOM处理后,每头大鼠预期反应可形成100-400个畸变小囊(Wangovich et al.(1992)

16G:51-54)。利用这些比率和95%置信度的卡方分析,当观察发现每头动物用试验药物处理后形成的畸变小囊数达到50-75个时,每组5只动物样本数量使得可以对试验动物进行统计学上显著抑制(P<0.05水平)的检测。
在按体重级别把大鼠分成测验组后,让实验动物适应10天。将试验药物溶于饮水中,浓度根据每天50ml的饮水量的平均值计算,使每头动物每天获得所需的剂量。在注射AOM之前,两组动物用药物处理一周,在第二周和第三周开始再通过两次皮下注射AOM,注射剂量为20mg/kg,诱导癌变的形成。然后再在第五周末给试验动物用药。
根据其他的在结肠炎模型方面的预临床研究,选择balsalazide用药剂量。一头500克大鼠,每天每公斤200毫克的剂量相当于一个70公斤体重的人每天用药14天,大约为活动性溃疡性结肠炎治疗剂量的两倍,为维持恢复期使用剂量的四倍以上。该剂量水平可使结肠中5-ASA的浓度为5-20mM。大鼠对balsalazide的最大耐受剂量超过12,000毫克/千克/天。
每周记录二次体重,每周测量两次每个笼子中动物的饮水量,饮水量按每个笼子中每个动物饮水的平均毫升数计算。
为了分析畸变小囊,在施药后的3周或5周末用CO2安死术处死大鼠。在直肠和肛门处切下大肠,取出大肠,并沿纵切面剖开。把大肠组织中的内含物清除干净,用盐溶液冲洗,在4℃下于2%的多聚甲醛溶液固定2小时,用0.2%亚甲蓝染色3-5分钟。再用盐溶液冲洗一次后,在40-100倍放大的显微镜下检查计数畸变小囊病灶的数量,同时测量结肠组织长度。在4℃下用乙二醇异丁烯酸盐包括经肉眼鉴别的病斑代表性样品;每个病灶3-4/um的切片用苏木精、曙红和苯胺蓝(HEA)染色,作组织学上计数检查。
为了分析胃十二指肠的糜烂,分别取出其基底、窦、和十二指肠,再分别按上述方法处理。根据描述的长度和严重度糜烂指数对胃糜烂进行记录(Peskar et al.(1986)

结果如图4所示,示出了两次皮下注射氧化偶氮基甲烷一周或三周后检查的大鼠每cm纵向长度的结肠组织中畸变小囊的平均数。在两个不同时间的检查中,用balsalazide处理的动物比对照动物形成的小囊平均数少得多。第1周对畸变小囊平均值的抑制率为61.8%,第3周的抑制率为58.1%。
对畸变小囊进程的抑制:图4中表示的数值取自于7-8cm结肠末端组织。在检查的早期点,该区域约含80%以上的总的畸变小囊病灶。因此,需再设计一个实验,在更长时间范围内,检查整个25cm的结肠组织,测定第二次注射AOM后,药物处理是否开始发生作用。此实验目的很有意义,因为当致癌物诱导的最初转化事件发生后,尤为重要的是证明化学预防剂对畸变小囊生长进程的抑制。
在此第二项研究中,8只动物分成一组,每组动物每周分二次注射氧化偶氮基甲烷。第二次注射24小时后,对一组动物开始用bal-salazide处理。六周以后分析检查畸变的小囊。大鼠在麻醉条件下,通过直肠滴注0.2%的亚甲基蓝,对小囊进行原位染色,然后再给大鼠去血,切除从肛门至盲肠的结肠组织,清洗用2%的多聚甲醛溶液固定2小时。在40x的放大镜下检查小囊数量。除计数病灶数量外,也检查每个病灶的畸变小囊的数量。如图6示出了这些研究结果。显然即使在第二次注射AOM后的开始的24小时,也观察到对畸变小囊病灶的极大抑制作用,该结果可与图4所示的原先的实验的结果相比较,总体上约60%的抑制作用。

13:1505-1512)用同样的癌症诱变模型研究报道了畸变小囊数量与最终肿瘤发展的相互关系。在其研究中,包含4个或更多的畸变小囊的病灶数量与肿瘤的形成非常相关,表明这些较大的病灶很可能发展为肿瘤。对上述实验结果进行分析,确定了balsalazide对畸变小囊数量(multi-plicity)的影响。这些结果如下面表II所示,balsalazide对含4个或更多畸变小囊的病灶数比含不到4个畸变小囊的病灶数的抑制作用更大。对含1-3个小囊的病灶,平均抑制率为57-61%(P<0.003,Fischer准确测验),而含4个或更多小囊的病灶数量平均减少72-76%(P<0.001)。当含4个或更多小囊的病灶数与含3个或以下的小囊的病灶数的抑制结果相互比较时,用Fischer准确测验法,在P<0.022水平上有显著差异。

实施例3:5-ASA的氧化产物在体外对结肠癌细胞增生的影响
虽然上述实验中balsalazide对两类细胞表现出相同的抑制作用,但LS174T细胞似乎始终比HT-29细胞对5-ASA的作用更加敏感。此外,两类细胞对5-ASA的抑制反应在个别单独的实验中有一定差异,最大变化程度为70%,最小为10%。在寻找变异的原因实验中,研究发现:在添加到细胞中前最少3天配制的5-ASA贮存液可一直产生较好的抑制效果。图7中显示出这种差异。
在该实验中,暴露于刚刚溶解的10mM5-ASA中的LS174T细胞4天后的生长率为对照细胞的82.7%。相比之下,暴露于在实验之前4天溶解于培养基的10mM5-ASA中的LS174T细胞4天后的生长率只有对照培养物的22.9%。这种显著差异说明5-ASA在有某种培养基成分存在时可被转变为一种更有活性的代谢物。
与5-ASA相关的重要活性之一是其抗氧化剂性质。的确,在肠炎治疗中的疗效被认为是由于它可以清除炎症细胞释放的自由基(包括O2 -),故而减少氧化损伤。这表明当5-ASA与组织培养基一起预培养时,观察发现的与5-ASA相关的抑制活性可能与一种发生的氧化事件相一致。为了验证这种可能性,用各种浓度的次氯酸钠使5-ASA发生氧化,然后再测定相应的对细胞生长的抑制活性。如下表III所示,增加次氯酸盐的用量可使5-ASA产生更大的抑制活性。

1.一种治疗结肠癌或有发展为结肠癌危险的患者的方法,该方法包括给患者施用一种有效量的一种药物组合物,该药物组合物包含具有下通式的2-羟基-5-苯基偶氮苯甲酸衍生物:
其中,X为-SO2-基或-CO-基,R为苯基或羧甲苯基或者是-(CH2)n-Y基团,其中Y是羟基、氨基、单烷基-或二烷基-氨基基团(其中的烷基部分含有的碳原子数可达6个)或羧基或磺酸基团,n为1至6个的整数,亚烷基上的一个或更多的氢原子可以被氨基、单烷基-或二烷基-氨基(其烷基部分包含的碳原子数可达6个)或烷基取代,其中-(CH2)n-Y基团可直接与氮原子相连,也可通过本环与氮原子相连,但-R-NH-X-不能是-CO-NH-CH2-COOH基团;或其酯或活性代谢物或活性代谢物的氧化产物;或者为2-羟基-5-苯基-偶氮苯甲酸衍生物非毒性的、药理学上可接受的盐或其酯或其活性代谢物或活性代谢物的氧化产物。
