- HINTERGRUND DER ERFINDUNG
- 1. Gebiet der Erfindung
- Die vorliegende Erfindung bezieht sich auf ein Gefäßtransplantat. Spezieller bezieht sich die vorliegende Erfindung auf eine zusammendrückbare Gefäßtransplantatstruktur mit flachem Profil zur Behandlung abdominaler oder anderer Aneurysmen.
- Gefäßaneurysmen sind das Ergebnis einer anomalen Erweiterung eines Blutgefäßes, die gewöhnlich durch krankheits- und/oder vererbungsbedingte Veranlagung verursacht wird, die die Wand einer Arterie schwächen und deren Ausdehnung zulassen kann. Aneurysmen können in jedem Blutgefäß, meistens in der Aorta und in den äußeren Arterien, auftreten, wobei die meisten aortischen Aneurysmen in der abdominalen Aorta auftreten, wobei sie unterhalb der renalen Arterien beginnen und sich häufig distal in eine oder beide Hüftarterien ausweiten.
- Aortische Aneurysmen werden gewöhnlich durch offene chirurgische Eingriffe behandelt, wobei das erkrankte Gefäßsegment durch einen Bypass umgangen wird und durch ein künstliches Gefäßtransplantat wiederhergestellt wird. Obwohl die herkömmliche Gefäßtransplantatchirurgie als eine wirksame chirurgische Technik anzusehen ist, leidet sie, insbesondere wenn die Möglichkeit eines meist gefährlichen aufgebrochenen abdominalen aortischen Aneurysmas berücksichtigt wird, unter einer Anzahl von Nachteilen. Der chirurgische Eingriff ist komplex und erfordert erfahrene Chirurgen und gut ausgestattete chirurgische Einrichtungen. Selbst mit den besten Chirurgen und der besten Einrichtung sind die häufig behandelten Patienten jedoch ältere Patienten, die durch cardiovaskuläre und andere Krankheiten geschwächt sind, wodurch sich die Zahl der in Frage kommenden Patienten verringert. Selbst für vor einem Bruch in Frage kommende Patienten ist die herkömmliche Beseitigung von Aneurysmen von einer relativ hohen Todesfallrate betroffen, die gewöhnlich zwischen 3 % und 10 % liegt. Die mit der herkömmlichen Chirurgie zusammenhängende Morbidität schließt den Herzinfarkt, das Nierenversagen, die Bewegungsunfähigkeit, die Lähmung und andere Zustände mit ein. Darüber hinaus nimmt die Wiedergenesung selbst bei erfolgreicher chirurgischer Operation mehrere Wochen in Anspruch und erfordert häufig einen längeren Krankenhausaufenthalt.
- Um einige oder sämtliche dieser Nachteile zu überwinden, wurde zur Behandlung von Aneurysmen das Einsetzen endovaskulärer Transplantate vorgeschlagen. Obwohl vielversprechend, unterliegen viele der vorgeschlagenen Verfahren und Vorrichtungen anderen Problemen. Häufig besitzen die vorgeschlagenen Transplantatstrukturen vorstehende Anker oder ein vorstehendes Gerüst, was thrombogen sein kann. Es ist außerdem schwierig, Transplantatstrukturen zu schaffen, die mit dem Blutgefäßlumen dicht abschließen, um insbesondere dann, wenn sie äußeren Verformungskräften unterworfen sind, die aus der Gefäßexpansion und -kontraktion mit dem Schlagen des Herzes resultieren, das Entweichen oder Umleiten von Blut in den geschwächten Aneurysmus zu verhindern. Viele Gefäßtransplantatstrukturen können sich nur schwer der Arterieninnenwand anpassen, insbesondere dann, wenn die Wand infolge einer Arteriosklerose und Verkalkung eine stark ungleichmäßige Oberfläche besitzt und mit dem Herzschlag und dem Blutfluß des Patienten ausdehnt und zusammenzieht. Zudem lassen sich viele herkömmliche Gefäßtransplantatstrukturen in der Zielregion des Gefäßes nur schwer positionieren und verankern.
- Aus diesen Gründen wäre es wünschenswert, wenn die Gefäßtransplantate in der Zielregion leicht zu plazieren wären, eine geringe oder keine Neigung zu Thrombosen aufweisen würden, zur Vermeidung eines Entweichens oder Umleitens von Blut einen dichten Abschluß mit der Gefäßwand schaffen würden und geeignet wären, sich gleichmäßigen und ungleichmäßigen Blutgefäßwänden anzupassen, selbst dann, wenn sich die Wand mit dem Herzschlag des Patienten ausdehnt und zusammenzieht.
- 2. Beschreibung des Standes der Technik
- Gefäßtransplantate und Vorrichtungen zu ihrem transluminalen Einsetzen sind beschrieben in den US-Patenten Nr.5.219.355, 5.211.658, 5.104.399, 5.078.726, 4.820.298, 4.787.899, 4.617.932, 4.562.596, 4.577.631 und 4.140.126 sowie in den europäischen Patentveröffentlichungen 508 473, 466 518 und 461 791.
- Expandierbare und selbstexpandierende Gefäßnetze sind beschrieben in den US-Patenten Nr. 5.147.370, 4.994.071 und 4.776.337, in den europäischen Patentveröffentlichungen 575 719, 556 850, 540 290, 536 610 und 481 365 und in der deutschen Patentveröffentlichung
DE 42 19 949 . - EP-A-05 568 50, gegen die die zweiteilige Form der Ansprüche 1 und 2 abgegrenzt ist, beschreibt eine spezielle Netzstruktur, die aus einem im Zickzack verlaufenden Draht gefertigt ist, der an benachbarten Scheitelpunkten verbunden ist, so daß eine kontinuierliche Helix gebildet wird. Die Netzstruktur ist zusammendrückbar und in eine vorkomprimierte Konfiguration selbstexpandierend und kann außerhalb oder innerhalb eines Transplantats, an das es mit Reifelementen befestigt wird, angeordnet werden.
- EP-A-05 40 290 offenbart ein expandierbares Netz, das aus mehreren radial expandierbaren zylindrischen Elementen besteht, wovon jedes aus einem bandförmigen Material gefertigt und durch wenigstens ein Zwischenverbindungselement verbunden ist. EP-A-551 179 offenbart ein Bypass-Transplantat mit separaten, in das expandierbare Transplantatmaterial eingelassenen röhrenförmigen Elementen.
- Eine elastische Gefäßnetzstruktur mit gegenläufig gewickelten schraubenlinienförmigen Elementen, wovon einige an bestimmten Stellen getrennt sind, um die Elastizität zu erhöhen, wird von Angiomed, Karlsruhe, Deutschland, vertrieben, wie dies in einer Broschüre mit dem Titel "Memotherm Iliaca Stents" beschrieben ist.
- Katheter zum Einsetzen von Gefäßnetzen sind beschrieben in den US-Patenten Nr.5.192.297, 5.092.877, 5.089.005, 5.037.427, 4.969.890 und 4.886.062.
- Gefäßtransplantate, die für die offene chirurgische Implantation vorgesehen sind, sind beschrieben in den US-Patenten Nr.5.236.447, 5.084.065, 4.842.575, 3.945.052 und 3.657.744 sowie in den PCT-Anmeldungen WO 88/00313 und WO 80/02641 und
SU 1697787 - Nickeltitan-Legierungen und ihre Verwendung in medizinischen Vorrichtungen sind beschrieben in den US-Patenten Nr. 4.665.906 und 4.505.767.
- ZUSAMMENFASSUNG DER ERFINDUNG
- Die vorliegende Erfindung umfaßt ein Gefäßtransplantat nach Anspruch 1 oder 2 zur Behandlung krankhafter Zustände, insbesondere von Aneurysmen. Gemäß bevorzugten Ausführungsformen umfassen die Gefäßtransplantate ein radial oder lateral zusammendrückbares perforiertes röhrenförmiges Gerüst mit einem proximalen Ende, einem distalen Ende und einem Lumen zwischen diesen Enden. Eine Innenverkleidung, üblicherweise ein Gewebe, eine Kunststoffschicht oder eine Membran oder dergleichen bedeckt die gesamte oder den größten Teil der Oberfläche des Lumens des röhrenförmigen Gerüsts und er streckt sich von einem Ort in der Nähe des proximalen Endes im Hohlraum zu einem Ort in der Nähe des distalen Endes. Die Verkleidung ist an wenigstens einem Ende sowie an mehreren Stellen zwischen den Enden an dem Gerüst befestigt. Optional kann über wenigstens einem Abschnitt der Außenseite des Gerüsts eine zweite Verkleidung vorgesehen sein, um beide Seiten des Gerüsts abzudecken. Eine solche Außenabdeckung sorgt für eine umfängliche Abdichtung gegen die Innenwand des Blutgefäßlumens, um ein Entweichen des Blutflusses zwischen dem Transplantat und der Lumenwand in den behandelten Aneurysmus oder die behandelte Stenose zu vermeiden.
- Die Transplantate der vorliegenden Erfindung finden besondere Anwendung bei der Behandlung vaskulärer Zustände wie etwa abdominaler oder anderer Aneurysmen, Gefäßstenosen und andere Zustände, die die Schaltung eines künstlichen Gefäßlumens erfordern. Zur Behandlung von Gefäßstenosen kann das Transplantat als Netz dienen, um die Gefäßdurchgängigkeit in ähnlicher Weise zu bewahren, wie es in den obenbeschriebenen US-Patenten und ausländischen Patentdokumenten, die sich auf Netze beziehen, beschrieben ist. Weitere intraluminale Anwendungen der vorliegenden Erfindung umfassen das Spannen des Harnleiters, der Harnröhre, des Gallentrakts und dergleichen und kann auch zur Schaffung vorübergehender oder langzeitiger Lumen verwendet werden wie etwa die Bildung einer Fistel.
- Solche Transplantatstrukturen besitzen mehrere Vorteile gegenüber den bisher vorgeschlagenen Entwicklungen, insbesondere für vaskuläre Anwendungen. Durch Abdeckung des Lumens des röhrenförmigen Gerüsts wird die Neigung des Transplantats, Thrombosen hervorzurufen-, die aus den vorstehenden Gerüstelementen resultiert, stark verringert oder beseitigt. Eine solche Verringerung der Neigung zu Thrombosen wird erzielt, obwohl die Vorteile, eine Gerüststruktur zu besitzen, die über das Transplantat hervorsteht, beibehalten werden. Ein solches Außengerüst unterstützt das Verankern des Transplantats an Ort und Stelle und bewahrt die Durchgängigkeit und Gleichmäßigkeit des Transplantatlumens, wobei beides ein Vorteil gegenüber Transplantatstrukturen darstellt, die nur an jedem Ende verankert und gestützt werden. Die bevorzugten Gefäßtransplantate der vorliegenden Erfindung sind außerdem selbstexpandierend und leicht einzusetzen. Die Selbstexpansionseigenschaft eines solchen Gerüsts wirkt außerdem äußeren Verformungskräften entgegen, die aus der ständigen physiologischen Expansion und Kontraktion des Blutgefäßlumens resultieren. Außerdem ermög licht das Fehlen von Treibkeilen, Zacken und anderen eindringenden Elementen auf dem Transplantat, daß das Transplantat sich enger an die umgebende Gefäßwand anpaßt, und erleichtert das Bergen und/oder das Neupositionieren des Transplantats, wie im folgenden näher beschrieben wird. Zusätzlich ermöglicht eine elastische röhrenförmige Gerüststruktur, daß sich das Transplantat selbst an unregelmäßige Bereiche der Blutgefäßwand anpaßt, wenn sich diese infolge der Pumpwirkung des Herzens des Patienten ausdehnt und zusammenzieht.
- Das röhrenförmige Gerüst enthält vorzugsweise mehrere radial zusammendrückbare Band- oder Ringstrukturen, wovon jede einen entspannten (d. h. nicht komprimierten) Durchmesser besitzt, der größer als der Durchmesser des zu behandelnden Blutgefäßes ist. Benachbarte zusammendrückbare Bandelemente sind jeweils voneinander unabhängig. Unabhängige Bandelemente halten durch ihre Befestigung an der (den) inneren und/oder äußeren Verkleidung (Verkleidungen) zusammen.
- Alternativ kann das röhrenförmige Gerüst mehrere seitlich zusammendrückbare axiale Elemente mit benachbarten axialen Elementen, die nicht direkt miteinander verbunden sind, enthalten. Die axialen Elemente enthalten gewöhnlich eine Vielzahl sich wiederholender Struktureinheiten, z. B. rhombenförmiger Elemente, die axial verbunden sind. Die axialen Elemente werden entweder durch Einnähen oder Einfassen der axialen Elemente in Taschen, die zwischen der Innenverkleidung und einer Außenverkleidung, die über dem Gerüst angeordnet ist, gebildet sind, an der inneren Verkleidung befestigt. Die Taschen können durch Befestigen der Innenverkleidung und der Außenverkleidung aneinander entlang axialer Linien zwischen benachbarten axialen Elementen gebildet sein.

- Die vorliegende Erfindung schafft ein Gefäßtransplantat, insbesondere zum transluminalen Einsetzen in das Gefäßsystem zur Behandlung von Aneurysmen und anderen vaskulären Zuständen, jedoch auch in andere Körperlumen wie etwa den Harnleiter, die Harnröhre, den Gallentrakt, den Magen-Darm-Trakt und dergleichen zur Behandlung anderer Zustände, denen das Einführen einer verstärkenden oder schützenden Struktur in dem Lumen Hilfe bringt. Das Gefäßtransplantat kann auch beim Schaffen von künstlichen Lumen durch feste Gewebe und Strukturen Anwendung finden. Die Gefäßtransplantate werden endovaskulär eingesetzt. So, wie es hier verwendet wird, bedeutet "endovaskulär" das Einsetzen durch perkutane oder chirurgische transluminale Eingriffe unter Verwendung eines unter dem Röntgenbildschirm geführten Katheters über einem Führdraht. Die Katheter und Führdrähte können über die gewöhnlichen Zugangsstellen in das Gefäßsystem wie etwa durch die Arterien im Arm oder unter dem Schlüsselbein zum Zugang zur Aorta und durch die Oberschenkelarterien zum Zugang zur Aorta oder zu Peripherie- und Nebenblutgefäßen.
- Ein Gefäßtransplantat gemäß der vorliegenden Erfindung enthält ein radial zusammendrückbares perforiertes röhrenförmiges Gerüst und eine Innen- oder Außenverkleidung, die in einem zentralen Lumen befestigt ist, das durch das Gerüst definiert ist, und optional eine zweite oder äußere Verkleidung, die über dem Äußeren des Gerüsts ausgebildet ist. Das radial zusammendrückbare Gerüst kann verschiedene Formen annehmen, wobei es gewöhnlich aus mehreren unabhängigen Strukturelementen wie etwa Ringen, Bändern, schraubenlinienförmigen Elementen, serpentinenartigen Elementen, axialen Verstrebungen, parallelen Stangen und dergleichen besteht, die aus einer entspannten größeren Durchmesserkonfiguration in eine Konfiguration mit kleinerem Durchmesser zusammengedrückt werden können, um, wie oben erläutert wurde, die Einführung zu erleichtern. Natürlich muß (müssen) die Verkleidung (Verkleidungen) an dem Gerüst sowohl in seiner radial zusammengedrückten Konfiguration als auch in seiner ausgedehnten, entspannten Konfiguration befestigt bleiben.
- Eine bevorzugte Konfiguration für das röhrenförmige Gerüst besteht aus mehreren radial zusammendrückbaren Bandelementen, wobei benachbarte Bandelemente nicht direkt miteinander verbunden sind. Statt dessen sind die Bänder nur mit der (den) Verkleidung (Verkleidungen) verbunden, die die axiale Integrität des Transplantats erhält (enthalten). Vorzugsweise sind die unabhängigen Bänder zwischen der Innenverkleidung und der Außenverkleidung angenäht oder versiegelt, wie weiter unten genauer beschrieben wird.
- In einer alternativen Konfiguration enthält das perforierte röhrenförmige Gerüst mehrere seitlich zusammendrückbare axiale Elemente, die direkt, z. B. durch Annähen, oder indirekt, z. B. durch Laminierung, an der Innenverkleidung befestigt sind. Die axialen Elemente können mehrere sich wiederholende Strukturelemente wie etwa Rhomben sein. Durch Positionierung der axialen Elemente in Taschen, die zwischen einer Innenverkleidung und einer Außenverkleidung gebildet sind, können die axialen Elemente einzeln nachgeben, wobei die gewünschte radiale Kompressibilität und die gewünschten Selbstexpansionseigenschaften für das Transplantat geschaffen werden.
- Die Abmessungen des röhrenförmigen Transplantats hängen von der beabsichtigten Verwendung ab. Üblicherweise besitzt das Transplantat eine Länge in der Größenordnung von ungefähr 50 mm bis 500 mm, für vaskuläre Anwendungen vorzugsweise im Bereich von 80 mm bis 200 mm. Der entspannte Durchmesser liegt gewöhnlich im Bereich von ungefähr 4 mm bis 45 mm, wobei er für vaskuläre Anwendungen vorzugsweise im Bereich von ungefähr 5 mm bis 25 mm liegt. Das Transplantat ist radial auf einen Durchmesser im Bereich von 3 mm bis 9 mm zusammendrückbar, für vaskuläre Anwendungen vorzugsweise im Bereich von 4 mm bis 6 mm.
- Die Verkleidung (Verkleidungen) setzt (setzen) sich aus herkömmlichen biologischen Transplantatmaterialien wie etwa Polyester, Polytetrafluorethylen (PTFE), Polyurethan und dergleichen zusammen, wobei sie gewöhnlich die Form von gewebten und ungewebten Stoffen, Polymerschichten, Membranen und dergleichen besitzen. Ein gegenwärtig bevorzugtes Gewebe-Verkleidungsmaterial ist ein glattes Polyestergewebe wie etwa Dacron®-Garn Typ 56 (Dupont, Wilmington, Delaware) mit einem Gewicht von 40 Denier, bei 27 Fäden, mit 178 Ketten pro Zoll in Umfangsrichtung und 78 Garnen pro Zoll in Füllrichtung gewebt.
- Die Verkleidung wird am Innenlumen des röhrenförmigen Gerüsts angebracht und bedeckt den größten Teil oder einen Teil der Innenfläche des Lumens. Beispielsweise kann die Verkleidung am röhrenförmigen Gerüst entlang mehrerer in Umfangsrichtung voneinander beabstandeter axialer Linien angenäht oder anderweitig befestigt werden. Durch eine solche Anbringung kann sich die Verkleidung beim radialen Zusammendrücken des Gerüsts entlang mehrerer axialer Faltlinien falten. Die Verkleidung kann sich ferner öffnen und der Lumenwand des röhrenförmigen Gerüsts anpassen, sobald sich dieses ausdehnt. Bei Verwendung einer Innenverkleidung und einer Außenverkleidung können die Verkleidungen alternativ angenäht oder durch Wärme oder Ultraschall verschweißt werden, so daß sie das röhrenförmige Gerüst zwischen sich einschließen. In einer exemplari schen Ausführungsform, bei der zwischen der Innenverkleidung und der Außenverkleidung mehrere unabhängige Bandelemente angeordnet sind, sind die Verkleidungen entlang Umfangslinien zwischen benachbarten Bandelementen aneinander befestigt, so daß sie Taschen zum Halten der Bandelemente bilden. In einer zweiten exemplarischen Ausführungsform, bei der zwischen der Innenverkleidung und der Außenverkleidung mehrere axiale unabhängige Bandelemente angeordnet sind, sind die Verkleidungen entlang axialer Linien zwischen benachbarten Bandelementen aneinander befestigt, um Taschen zum Halten der Bandelemente bilden.
- Die Verkleidung schließt vorzugsweise an wenigstens einem Ende und im Fall von nicht gabelförmigen, geraden Transplantaten vorzugsweise an beiden Enden umfänglich gegen das röhrenförmige Gerüst ab. Außerdem wird in manchen Fällen vorgezogen, das distale und das proximale Ende der des perforierten röhrenförmigen Gerüst üblicherweise über eine Länge im Bereich von ungefähr 1 mm bis 25 mm vorstehen zu lassen, d. h., nicht mit dem Verkleidungsmaterial zu bedecken. Ein nicht mit der Verkleidung bedecktes Gerüst ermöglicht das Durchströmen von Blut durch die Perforationen in die Nebenarterien wie etwa die renalen Arterien im Fall von Transplantaten für eine abdominale Aorta, wobei für das Verankern des Gerüsts im Blutgefäßlumen eine zusätzliche Fläche geschaffen wird. In einer exemplarischen Ausführungsform erstreckt sich die Verkleidung durch das Gerüst über die Außenfläche in die Nähe entweder eines oder beiden Enden, so daß ein wirksamerer Abschluß mit der benachbarten Blutgefäßwand geschaffen wird.
- Das radial zusammendrückbare perforierte röhrenförmige Gerüst besteht aus einem elastischen Material, gewöhnlich aus einem Metall, häufig aus einer wärme- und/oder formbeständigen Metall, wie etwa Nickeltitaniumlegierung, die unter dem Handelsnamen Nitinol® im Handel erhältlich ist. Die Gerüste können auch aus anderen hochelastischen Metallen wie etwa MP-35 N, Elgiloy, 316 L rostfreiem Stahl 316 L und dergleichen zusammengesetzt sein. Im Fall von Nitinol® und anderen Memory-Legierungen kann der Phasenübergang zwischen dem austenitischen und dem martensischen Zustand zwischen einer Einführungstemperatur, z. B. der Raumtemperatur (ungefähr 22 °C), und der Körpertemperatur (37 °C), eintreten, um die Belastung des nichtexpandierten Gerüst zu minimieren und die radiale Expansion des Gerüsts aus ihrem radial zusammengedrückten Zustand zu verbessern. Außerdem kann aufgrund der hochelastischen Natur der Legierung eher eine Expension, als eine Wiedergewinnung der eigentlichen Form aufgrund einer Phasenänderung erzielt werden.
- In manchen Fällen ist es wünschenswert, ein röhrenförmiges Gerüst zu bilden, das in verschiedenen Regionen entlang ihrer Länge verschiedene elastische oder andere mechanische Eigenschaften aufweist. Beispielsweise können verschiedene Regionen des röhrenförmigen Gerüsts wärmebehandelt werden, so daß einige Regionen elastische Eigenschaften besitzen, während andere formbar werden, so daß sie durch äußere Kräfte verformbar werden. Beispielsweise kann das Transplantat durch Schaffung wenigstens eines formbaren Endabschnitts und eines elastischen (radial zusammendrückbaren) Mittelabschnitts durch innere Ballon-Expansionskräfte (um das Ende bzw. die Enden in der Innenwand des Blutgefäßes zu verankern) starr aufgeweitet und eingesetzt werden, wobei das mittlere aufgrund der elastischen Natur des röhrenförmigen Elements offen bleibt. Formbare Endabschnitte sind von besonderem Vorteil, da sie mit einer ausreichenden Kraft expandiert werden können und erneut expandiert werden können, falls dies erforderlich ist, um eine gute Abdichtung gegen die Blutgefäßwand sicherzustellen. Alternativ könnten die formbaren Enden aus einem von jenem des Mittelabschnitts des röhrenförmigen Gerüsts verschiedenen Material gebildet sein. Eine Verwendung verschiedener Materialien wäre besonders zweckmäßig, wenn das Gerüst aus mehreren unabhängigen Bändern gebildet ist, wobei eines oder mehrere Bandelemente an einem oder beiden Enden aus einem formbaren Metall geformt sein könnten. Gewöhnlich erstreckt (erstrecken) sich ein solches formbares Ende (solche formbaren Enden) über eine Strecke im Bereich von 5 mm bis 50 mm, vorzugsweise von 5 mm bis 20 mm.
- Formbare Abschnitte oder Segmente können auch in anderen Teilen des röhrenförmigen Gerüsts ausgebildet sein. Beispielsweise könnten einige in Umfangsrichtung beabstandete Segmente des röhrenförmigen Gerüsts formbar sein, während die restlichen Umfangssegmente elastisch wären. Das Gerüst würde somit elastisch bleiben, hätte jedoch eine zusätzliche Formbarkeit, um durch Anlegen einer inneren Expansionskraft die Expansion zu erlauben. Eine solche Konstruktion wäre vorteilhaft, da es möglich wäre, den Durchmesser der Transplantat- oder Netzstruktur zu erweitern, wenn der Innendurchmesser (der insgesamt aus der elastischen Expansion hervorging) aus irgendeinem Grund nicht groß genug wäre. Das Verhältnis von elastischem Material zu formbarem Material in dem röhrenförmigen Gerüst kann so gewählt werden, daß ein gewünschtes Gleichge wicht zwischen dem Maß an ursprünglicher, elastischer Öffnung und der Verfügbarkeit einer zusätzlichen, formbaren Öffnung hergestellt wird. Eine solche Konstruktion kann durch selektive Wärmebehandlung von Abschnitten eines aus einem einzigen Legierungsmaterial, z. B. einer Nickeltitaniumlegierung, zusammengesetzten Gerüsts oder durch Bildung von Umfangssegmenten des Gerüsts aus verschiedenen Materialien mit verschiedenen elastischen/formbaren Eigenschaften erreicht werden. Insbesondere könnten einzelne seitlich zusammendrückbare axiale Elemente
204 (die im Zusammenhang mit1B beschrieben wurden) aus Materialien mit verschiedenen elastischen/formbaren Eigenschaften gebildet werden. - Mit Bezug auf
1 wird nun eine exemplarische Transplantatstruktur10 beschrieben, die konstruktionsgemäß dem Gerüst des Transplantats gemäß der Erfindung ähnlich ist, mit Ausnahme, daß benachbarte Bandelemente miteinander verbunden sind, während gemäß der Erfindung benachbarte Bandelemente oder lineare Elemente nicht direkt miteinander verbunden sind. Die Transplantatstruktur10 umfaßt eine Gewebeverkleidung12 und ein radial zusammendrückbares perforiertes röhrenförmiges Gerüst14 . Das Gerüst ist in jeder dieser Figuren in seiner expandierten (entspannten Konfiguration) gezeigt, wobei es jedoch durch Anlegen einer radial nach innen gerichteten Druckkraft radial zusammengedrückt werden kann, wobei das Transplantat10 gewöhnlich in einer Außenhülle angeordnet ist, wie im folgenden näher beschrieben wird. - Das röhrenförmige Gerüst
14 enthält mehrere radial zusammendrückbare Bandelemente11 , wovon jedes ein zickzackförmiges oder Z-förmiges Element enthält, das einen kontinuierlichen kreisförmigen Ring bildet. Jedes Bandelement11 besitzt typischerweise eine Weite w im Bereich von 2 mm bis 15 mm, wobei das röhrenförmige Gerüst zwischen 1 und 30 einzelne Bandelemente enthält. Benachbarte Bandelemente11 sind vorzugsweise voneinander um einen kleinen Abstand g beabstandet und durch Brückenelemente13 verbunden. Die Elastizität wird dadurch verbessert, daß zwischen jedem benachbarten Paar von Bandelementen11 nur zwei diametral entgegengesetzte Brückenelemente13 geschaffen sind. Wie ferner mit Bezug auf1A beschrieben wird, kann die Flexibilität weiter verbessert werden, wenn die Bandelemente so belassen werden, daß sie nur durch die Verkleidung verbunden sind. - Gewöhnlich bleibt das perforierte röhrenförmige Gerüst
14 an jedem Ende offen, z. B. bleibt wenigstens ein Teil des letzten Bandelements11 von der Ver kleidung12 unbedeckt. Die Verkleidung12 wird an den Bandelementen11 , vorzugsweise an den Verbindungen oder Knoten, an denen die das Element die Richtung wechselt, um das Z-Muster zu bilden, angenäht oder auf andere Weise befestigt (wobei das Nähen zwischen den Bandelementen nicht überkreuz geschehen sollte, derart, daß die Elastizität eingeschränkt wäre). Die Verkleidung12 verläuft gewöhnlich außerhalb des inneren Lumens des röhrenförmigen Gerüsts14 durch die Lücke zwischen benachbarten Bandelementen zur Außenseite des Gerüsts, wie dies in1 gezeigt ist. Der Abschnitt der Verkleidung12 auf der Außenseite des röhrenförmigen Gerüsts14 unterstützt die Abdichtung der Enden des Transplantats10 gegen die Wand des Blutgefäßes oder eines anderen Körperlumens, indem es angeordnet ist. - Die Expension ist bei 30° gezeigt, erstreckt sich bei der Anwendung jedoch häufig bis zu 60° oder mehr.
- Ein bevorzugtes Verfahren zur Bildung des röhrenförmigen Gerüsts
14 ist wie folgt. Eine Röhre aus dem gewünschten elastischen Material wie etwa Nickeltitaniumlegierung mit einer Phasenübergangstemperatur, die wesentlich unter 37 °C, vorzugsweise zwischen 30 °C und 32 °C, liegt, wird erhalten. Die Röhre besitzt Dimensionen, die ungefähr gleich den gewünschten Dimensionen des Gerüsts, wenn es radial zusammengedrückt ist, entsprechen. Die Röhre kann gezogen, gewalzt oder anderweitig behandelt werden, um die gewünschte Wanddicke, den gewünschten Durchmesser usw. zu erhalten. Geeignete Wanddicken liegen im Bereich von ungefähr 0,1 mm bis 0,5 mm. Ein Muster von axialen Schlitzen wird danach in der Röhre ausgebildet. Die Schlitze können durch Bearbeitung durch Elektroerosion (EDM), photochemisches Ätzen, Laserschneiden, maschinelle Bearbeitung oder andere herkömmlichen Techniken ausgebildet werden. Sobald die Schlitze ausgebildet worden sind, wird die Röhre in ihre gewünschte endgültige (entspannte) Durchmesserform mechanisch ausgeweitet und bei einer geeigneten Temperatur wärmebehandelt, um das Gerüst in den gewünschten expandierten Zustand zu versetzen. Scharte Kanten werden durch herkömmliche Techniken wie etwa Entgraten, Schleifpressen oder dergleichen entfernt. Das Ergebnis der Expansion ist das in1 gezeigte röhrenförmige Gerüst. - Vorzugsweise wird jedes Ende der Verkleidung
12 in Umfangsrichtung bei oder in der Nähe des distalen und des proximalen Endes des röhrenförmigen Transplantats versiegelt. Wie in1A gezeigt ist, kann dies durch Falten über das Ende der Verkleidung12 auf die Außenfläche des Transplantats10 erreicht werden. Zweckmäßigerweise kann dies durch die Lücken, die zwischen benachbarten Bandelementen14 vorhanden sind, geschehen. Wo die Verbindungen13 bleiben, kann die Verkleidung12 sorgfältig auf die Unterschicht des Gerüsts angenäht werden, wie bei18 in1A gezeigt ist. Weitere Techniken für das umfängliche Versiegeln der Verkleidung umfassen das Wärme- oder Ultraschall-Verschweißen der Verkleidung, das Laminieren einer äußeren Dichtung, das Aufnähen eines äußeren Verstärkungselements oder dergleichen. - Mit Bezug auf
1A wird nun eine exemplarische Ausführungsform 100 eines nach den Prinzipien der vorliegenden Erfindung hergestellten Gefäßtransplantats beschrieben. Das Transplantat100 besteht aus einem perforierten röhrenförmigen Gerüst102 , das mehrere unabhängige (nicht verbundene) Bandelemente104 enthält, die voneinander durch Lücken106 getrennt sind. Das perforierte röhrenförmige Gerüst102 ähnelt in der Konstruktion dem Gerüst14 des Transplantats10 , mit Ausnahme, daß benachbarte Bandelemente nicht direkt miteinander verbunden sind. Die Bandnummern104 sind nur durch eine Innenverkleidung108 und eine Außenverkleidung110 verbunden, wobei die Innenverkleidung und die Außenverkleidung zusammen die andernfalls frei schwebenden Bandelemente104 umfassen oder sandwichartig einhüllen. Um die Bandelemente104 an Ort und Stelle zu halten und die Verkleidungen an dem perforierten röhrenförmigen Gerüst102 zu befestigen, sind die Innenverkleidung und die Außenverkleidung entlang Umfangslinien112 miteinander verbunden, die vorzugsweise in den Lücken106 zwischen benachbarten Bandelementen104 verlaufen. Die Verkleidungen können durch Nähen, Wärmeschweißen, Ultraschallschweißen oder dergleichen miteinander verbunden werden. In der exemplarischen Ausführungsform bestehen die Verkleidungen108 und110 aus einem Polymerschichtmaterial und werden durch Ultraschallschweißen miteinander verbunden. Die Bandelemente104 müssen ferner an jedem Ende des Transplantats100 mit den Verkleidungen108 und110 befestigt werden. Sie könnten beispielsweise angenäht, verschweißt oder anderweitig mit den Verkleidungen verbunden sein, um sie an Ort und Stelle zu halten. Die Dimensionen, Materialien und andere Aspekte des Transplantats100 sind allgemein dieselben wie jene, die zuvor für das Transplantat10 beschrieben wurden. - Mit Bezug auf
1B wird nun eine alternative exemplarische Ausführungsform200 des Gefäßtransplantats der vorliegenden Erfindung gezeigt. - Das Transplantat
200 enthält ein perforiertes röhrenförmiges Gerüst202 mit mehreren seitlich zusammendrückbaren axialen Elementen204 . Jedes axiale Element204 enthält mehrere rhombenförmige Strukturelemente, die linear miteinander verbunden sind. Es sei angenommen, daß jedes rhombenförmige Strukturelement seitlich zusammendrückbar ist, so daß das Gerüst202 als Ganzes aus einer Konfiguration mit kleinerem Durchmesser in eine Konfiguration mit einem ausgedehnten Durchmesser gepreßt werden kann. Wie in1B gezeigt ist, befindet sich das Gerüst in einer teilweise komprimierten Konfiguration. Die axialen Elemente202 sind zwischen einer Innenverkleidung206 und einer Außenverkleidung208 eingefaßt. Die Innenverkleidung206 und die Außenverkleidung208 sind entlang mehrerer axialer Linien210 , die zwischen benachbarten axialen Elementen204 verlaufen, befestigt. Auf diese Weise ist jedes axiale Element204 in einer zwischen der Innenverkleidung206 und der Außenverkleidung208 gebildeten Tasche eingefangen. Wie bei der anderen Ausführungsform können sich die Enden des Gerüsts über die Verkleidungen hinaus erstrecken, so daß eine bessere Verankerung und Durchblutung auf beiden Seiten des Transplantats geschaffen wird.
Claims (6)
- Gefäßtransplantat, umfassend: ein perforiertes selbstexpandierendes Gerüst (
102 ) mit einem proximalen Ende, einem distalen Ende und einem Lumen dazwischen, und eine innere Verkleidung (108 ), die sich von einem Ort in der Nähe des proximalen Endes im Lumen zu einem Ort in der Nähe des distalen Endes im Lumen erstreckt, wobei die Verkleidung (108 ) das Lumen des röhrenförmigen Gerüsts (102 ) über die gesamte Strecke von dem Ort in der Nähe des proximalen Endes zu dem Ort in der Nähe des distalen Endes abdeckt, dadurch gekennzeichnet, dass das Gerüst (102 ) eine elastische röhrenförmige Gerüststruktur hat, die mehrere benachbarte, radial zusammendrückbare, Reifelemente (104 ) aufweist, die nicht direkt miteinander verbunden sind, und wobei die Reifelemente (104 ) ein zick-zack-förmiges oder Z-förmiges Element enthalten, das einen kontinuierlichen kreisförmigen Ring bildet, und unabhängig voneinander an der inneren Verkleidung (108 ) befestigt sind, die dadurch den axialen Zusammenhalt des Transplantats aufrecht erhält. - Gefäßtransplantat, umfassend: ein perforiertes selbstexpandierendes Gerüst (
202 ) mit einem proximalen Ende, einem distalen Ende und einem Lumen dazwischen, und eine innere Verkleidung (206 ), die sich von einem Ort in der Nähe des proximalen Endes im Lumen zu einem Ort in der Nähe des distalen Endes im Lumen erstreckt, wobei die Verkleidung (206 ) das Lumen des röhrenförmigen Gerüsts (202 ) über die gesamte Strecke von dem Ort in der Nähe des proximalen Endes zu dem Ort in der Nähe des distalen Endes abdeckt, dadurch gekennzeichnet, dass das Gerüst (202 ) eine elastische röhrenförmige Gerüststruktur hat, die mehrere benachbarte, lateral zusammendrückbare axiale Elemente (204 ) aufweist, die nicht direkt miteinander verbunden und voneinander durch Lücken (106 ) getrennt sind, und wobei die axialen Elemente (204 ) mehrere sich wiederholende Struktureinheiten enthalten, die axial verbunden sind, wobei die axialen Elemente (204 ) unabhängig voneinander an der inneren Verkleidung (206 ) befestigt sind, die dadurch den radialen Zusammenhalt des Transplantats aufrecht erhält. - Gefäßtransplantat nach Anspruch 2, wobei die axialen Elemente (
204 ) jeweils mehrere diamantförmige Elemente aufweisen. - Gefäßtransplantat nach einem der Ansprüche 1 bis 3, wobei die Verkleidung (
108 ,206 ) an wenigstens einem der Orte in der Nähe des proximalen Endes und in der Nähe des distalen Endes an dem perforierten röhrenförmigen Gerüst (102 ,202 ) in Umfangsrichtung befestigt ist und von einem Ende des perforierten röhrenförmigen Gerüsts (102 ,202 ) um eine Strecke im Bereich von 1 mm bis 20 mm nach innen beabstandet ist, wobei wenigstens ein Endabschnitt des perforierten röhrenförmigen Gerüsts (102 ,202 ) unabgedeckt bleibt. - Gefäßtransplantat nach einem der Ansprüche 1 bis 4, wobei die unabhängigen Reifelemente (
104 ) oder die axialen Elemente (204 ) an die Verkleidung (108 ,206 ) genäht sind. - Gefäßtransplantat nach einem der Ansprüche 1 bis 5, wobei die Reifelemente (
104 ) oder die axialen Elemente (204 ) an der inneren Verkleidung (108 ,206 ) durch eine mit der inneren Verkleidung (108 ,206 ) befestigte äußere Verkleidung (110 ,208 ) befestigt sind.
